All quotations subject to verification as to the compatibility of equipment ordered and to credit approval by the Home Office. The terms and conditions on the reverse side hereof are part of this quotation and such terms and conditions shall govern the sale of equipment and services hereunder, whether or not such order is placed on Customer’s Purchase Order or pursuant to this form. Customer has read and understands the terms and conditions printed on the reverse side of this form and attachment A hereto and agrees that such terms and conditions shall govern the sale of the equipment and services set forth in this quotation and shall govern the sale of the equipment and services set forth in this quotation and order sheet when Customer order is accepted. Prices are firm for requested shipment of equipment and services within 4 months of order date. Shipments requested by buyer more than 4 months, up to 9 months from order date, are subject to increase to then current published price levels, with a maximum increase of 5%. All special orders are subject to restock fee up to 25% for equipment not received and up to 90% for equipment or labor in sellers’ possession. Credit card payments are accepted on a NET 10 invoice payment. After 10 days credit card fee’s shall be applied to the invoice total. Fees shall not to exceed 5%.
